Student visa for Malaysia?

If you have an unconditional offer from a university and the university if applying for the student visa/p on your behalf, normally how long does it take to get that letter of approval from Malaysian ministry of higher education and how likely is it, that you application for student visa despite of having the unconditional offer gets rejected.
